LI-RADS stands for Liver Imaging Reporting and Data System. It turns what a radiologist sees on a liver scan into a single category, from definitely benign to definitely cancer.

It is deliberately narrow. LI-RADS applies to adults with cirrhosis not caused by a vascular condition, to people with chronic hepatitis B with or without cirrhosis, and to people with current or prior hepatocellular carcinoma. The restriction is deliberate. The scoring only holds up when the underlying risk of liver cancer is already high. Applied to a healthy liver, the same imaging patterns would flag too many harmless spots.

Why a scan can diagnose this cancer without a biopsy

Hepatocellular carcinoma, or HCC, has an unusual property. It behaves distinctively on contrast imaging, because it draws its blood supply from the hepatic artery while the surrounding liver is fed mostly by the portal vein.

So the tumor lights up early, then fades. NCI describes the standard test as a triple-phase, contrast-enhanced dynamic CT or MRI. For tumors 1 to 3 cm in a person at risk, a single study showing that pattern carries 95% to 100% specificity. That is high enough to diagnose without tissue.

A StatPearls chapter reports that LR-4 and LR-5 together reached 96.4% specificity and 65.4% sensitivity for HCC, including in lesions under 2 cm. High specificity, moderate sensitivity: the categories rarely mislabel a benign spot as cancer, but they do miss some cancers.

Size. The largest diameter, which changes what the other features mean.

What the numbers mean

The rule that surprises people

An observation under 10 mm cannot be called LR-5, no matter how classic it looks.

Small lesions are too easy to over-call. NCI describes the standard response for a lesion under 1 cm found during surveillance: close follow-up at 3-month intervals, using the same technique that found it.

So a small bright spot on your report is not a diagnosis being withheld. It is a spot that current imaging cannot resolve, being watched on a defined schedule.

What happens after each category

The category drives the next step rather than the treatment itself.

  • LR-1 and LR-2: return to routine surveillance.
  • LR-3: repeat or alternative imaging, often within a few months.
  • LR-4: more imaging, a biopsy, or a multidisciplinary review.
  • LR-5: treatment planning begins, usually without biopsy.
  • LR-M: biopsy, because the cancer type must be established.

NCI notes that biopsy may be considered when two imaging tests fail to characterize a lesion larger than 1 cm in a high-risk patient.

Size and number then feed transplant decisions. NCI states the Milan criteria as a single HCC lesion smaller than 5 cm, or 2 to 3 lesions each smaller than 3 cm.

For staging, NCI describes the Barcelona Clinic Liver Cancer system as the most accepted. It combines tumor stage, liver function, physical status, and symptoms. The AJCC TNM system is less useful here, because it does not account for how well the liver is working.

Categories are not permanent

Ancillary features can nudge a category up or down. Published examples include restricted diffusion, moderate T2 brightness, fat inside the lesion, corona enhancement, and a mosaic internal pattern.
