What lytic means
Lytic means broken down or dissolved. On an X-ray, CT, or bone scan, a lytic lesion is a spot where bone looks less dense than normal, sometimes described as a hole or a punched-out area. This happens when cells called osteoclasts destroy bone faster than the body can rebuild it. It is the opposite of a sclerotic lesion. There, extra bone builds up instead.
Why cancer can cause this
Cancer that spreads to bone can trigger this destructive process. Breast, lung, and kidney cancers are the ones usually classed as lytic. So is multiple myeloma. Prostate cancer sits at the other end, usually producing the dense, sclerotic kind instead. Breast cancer can also produce a mix of both patterns.
Why lytic lesions raise fracture risk
This is a detail that makes lytic lesions a bigger practical concern than sclerotic ones, in many cases. The lesion is actual bone loss, not extra bone. Because of that, the structure of the bone weakens right where the lesion sits. This raises the risk of a pathologic fracture. That means a break caused by weakened bone, rather than a major injury. Sometimes this happens with only minor stress. A fall from standing height can do it. So can normal daily activity. Doctors watch weight-bearing bones especially closely for this reason. The spine, hips, and thighbones matter most, since a fracture in one of these spots can be disabling.
Lytic lesions are not always cancer
A lytic-looking spot is not automatically cancer. Benign bone cysts can create a similar appearance. So can certain non-cancerous bone tumors, infections, and areas of prior trauma. This is part of why radiologists describe a lytic lesion's edges carefully. A well-defined border, with a narrow zone of transition into normal bone, tends to look more benign. A poorly defined, ragged border, with a wide zone of transition, tends to look more concerning for cancer.
What a lytic lesion does not tell you
Finding a lytic lesion does not by itself tell you whether it is cancer. If it is, it does not tell you which cancer caused it. It also does not tell you, on its own, how likely a fracture is at that exact spot. Its size and location factor heavily into that risk, though. Your team weighs the lesion's appearance alongside your cancer history and other scan findings. Sometimes a biopsy is needed for a fuller answer.

When to get help sooner
- Call 911 or go to an emergency department if you have new weakness or numbness in your legs, trouble walking, or you lose control of your bladder or bowels. With back pain and a lytic lesion in the spine, these point to pressure on the spinal cord. The American Cancer Society calls this an emergency, because untreated it can lead to paralysis.
- Call 911 or go to an emergency department if a bone gives way suddenly. That means sudden severe pain in a limb, hip, or back after minor stress, or being unable to put weight on a leg.
- Call your care team the same day if you become confused or unusually drowsy, are very thirsty and passing large amounts of urine, or cannot keep food down because of nausea and vomiting. These can be signs of a high blood calcium level, which lytic bone destruction can cause.
- Call your care team within a day or two if bone pain is steadily getting worse, wakes you at night, or is no longer controlled by the medicine you were given.
