FDA Approval: Pertuzumab (Perjeta) for Breast Cancer

Two antibodies, one receptor, two different grips

Pertuzumab, sold as Perjeta, carries an Initial U.S. Approval date of 2012 on its FDA label. Its label describes it as a HER2/neu receptor antagonist.

HER2 is a protein on the surface of breast cells that tells them to grow. In roughly one breast cancer in five, the tumor makes far too much of it. Those cancers are called HER2 positive.

Trastuzumab, an older antibody, binds one part of the HER2 receptor. Pertuzumab binds a different part, the site where HER2 pairs up with a related receptor called HER3. Blocking that pairing shuts down a signal trastuzumab alone leaves open. That is why the two are given together rather than one instead of the other.

Three places in the treatment path

The label names three uses, and they sit at very different points in a person's care.

Metastatic disease. With trastuzumab and docetaxel, for adults with HER2-positive metastatic breast cancer who have not had prior anti-HER2 therapy or chemotherapy for metastatic disease.

Before surgery. With trastuzumab and chemotherapy, as neoadjuvant treatment for adults with HER2-positive breast cancer that is locally advanced, inflammatory, or early stage, where the tumor is larger than 2 centimeters or the lymph nodes are involved. Neoadjuvant means given before surgery to shrink the tumor. The label frames it as part of a complete regimen for early breast cancer, not a standalone course.

After surgery. As adjuvant treatment for adults with HER2-positive early breast cancer at high risk of recurrence.

The label also states that HER2 testing must be performed using FDA-approved tests in qualified laboratories. Without that result, none of these uses apply. Our page on biomarker testing explains what those tests measure.

The warnings that shape monitoring

Perjeta carries a boxed warning, the FDA's strongest warning format, naming two problems.

Left ventricular dysfunction. The drug can cause both silent and symptomatic heart failure, showing up as a fall in left ventricular ejection fraction, or LVEF. LVEF is a measure of how much blood the heart's main pumping chamber pushes out with each beat. The label instructs that cardiac function be checked before treatment and during it, and that the drug be stopped for a confirmed, clinically meaningful drop.

That is why people on this regimen have echocardiograms on a schedule. It is not a formality.

Embryo-fetal toxicity. Exposure can cause fetal death and birth defects. The label requires that patients be advised of the risk and of the need for effective contraception.

The drug is given by intravenous infusion only, and the label specifies it must not be given as a rapid push.

When to get checked

Screening in the United States is a mammogram every other year for women aged 40 through 74.

Screening finds cancer before symptoms. Some HER2-positive cancers, and inflammatory breast cancer in particular, do not wait for a scheduled mammogram. Contact a clinician promptly for:

  • A new lump or thickened area in the breast or armpit.
  • Skin on the breast that becomes red, warm, swollen, or looks like orange peel.
  • A breast that changes size or shape over weeks.
  • Skin dimpling or puckering.
  • A nipple that turns inward, or new scaling and crusting.
  • Clear or bloody discharge from one nipple.

The redness-and-swelling picture matters because inflammatory breast cancer is often mistaken for infection at first. It develops over weeks, not months, and it is named in the pertuzumab label as a setting for neoadjuvant treatment.

How HER2 status is established

The path to a HER2 result runs through tissue. Imaging finds the abnormality. A biopsy takes a sample. A pathologist then runs tests for estrogen receptor, progesterone receptor, and HER2.

Staging follows, using tumor size, whether lymph nodes under the arm contain cancer, and whether the disease has spread further. Receptor status and stage together define the plan. Two people with identical stages and different receptor results receive quite different treatment.

Local treatment, systemic treatment

NCI divides breast cancer treatment into local and systemic. Local treatments, surgery and radiation, are aimed at the area with cancer. Systemic treatments are drugs that reach cancer cells throughout the body.

Surgery is either lumpectomy, removing the tumor and a rim of tissue, or mastectomy, removing the breast. Radiation often follows lumpectomy. Systemic options listed by NCI are chemotherapy, hormone therapy, immunotherapy, and targeted therapy.

For HER2-positive disease, targeted anti-HER2 drugs are the backbone, usually with chemotherapy. The neoadjuvant use of pertuzumab means some of that treatment happens before the operation rather than after, which also gives the team a direct readout: how much cancer remains in the tissue removed at surgery.

Population numbers, and what they omit

These SEER figures describe the whole US population of women with breast cancer, across all subtypes. HER2-positive disease is one subtype within that, so these numbers do not describe it specifically, and they describe no individual.

Five-year relative survival for female breast cancer is 91.9 percent for cases from 2016 to 2022. By stage it is 100.0 percent while the cancer is confined to the breast, 87.5 percent once it involves nearby lymph nodes, and 33.8 percent once it has spread to distant organs. About 64 percent are found at that first stage. The American Cancer Society projects 321,910 new cases and 42,140 deaths for 2026, and median age at diagnosis is 64.

What this approval cannot tell you

A 2012 approval date marks the first indication, for metastatic disease. The neoadjuvant and adjuvant uses came later, and they changed who receives the drug far more than the original approval did.

An approval also does not decide anything for one person. HER2 status, stage, heart function, pregnancy plans, and other treatments all feed into whether this regimen fits.
