Your blood marker has gone up. Your scan looks the same as last time. You are being told to wait and repeat.

This combination is well recognized in oncology clinics, and it is one of the harder places to sit. The blood test suggests something may be changing; the imaging offers nothing to point at. Small-volume disease can be present and shedding protein while imaging remains formally unchanged.

Treatment itself may be the cause. Early in chemotherapy some markers surge before falling, as cancer cells break down. Testing in the weeks immediately before or after a change of therapy can produce numbers that mislead.

Why teams usually watch rather than act

Changing treatment on a single rising value carries real risk. If the rise turns out to be benign or spurious, you will have stopped something that was working and moved earlier into your next line of therapy, reducing what remains available later.
