Cells throughout your body die all the time. As they die, they release fragments of their DNA into the bloodstream. Most of that free-floating DNA comes from normal cells. When a tumor is present, some of it comes from the tumor. That portion is circulating tumor DNA, or ctDNA.

A liquid biopsy is a blood draw that captures this free DNA and sequences it. It looks for the genetic changes that identify cancer. More usefully, it looks for the ones that a drug can target. Despite the name, nothing is cut and nothing is removed.

Where it genuinely helps

When there is not enough tissue. A small needle biopsy may bring back too few cells for a full genomic panel. Rather than repeat the procedure, doctors can often get the answer from blood.

The limitation that matters most

A negative liquid biopsy does not mean a mutation is absent. It may mean the tumor is not shedding enough DNA for the test to see it.

Shedding varies enormously. Small tumors shed little. Brain tumors shed poorly into blood, because of the blood-brain barrier. Some tissue types and some body sites release far less than others.

A result in one of these genes may reflect your bone marrow rather than your cancer. Laboratories flag this more and more often. The emerging use: minimal residual disease

The most active area of development uses ctDNA after treatment. The aim is to detect cancer that remains but is too small to see on any scan. Detectable ctDNA after surgery is consistently linked to a higher risk of recurrence. The evidence is furthest along in colorectal cancer. There, trials have tested whether ctDNA results can decide who needs chemotherapy after surgery.

This is genuinely promising, and genuinely unfinished. Knowing that a test predicts recurrence is not the same as knowing that acting on it earlier helps people live longer or better. Outside specific settings and clinical trials, most teams will not change treatment on an MRD result alone.
